Community Needs Assessment Survey

The Lubbock Public Health Department in conjunction with Lubbock Area United Way is conducting a comprehensive needs assessment to measure medical and social factors that impact health in our community. We want the final product to be of value to agencies throughout the community. Before starting the assessment process, we want to identify the specific data needs of our community partners and friends to identify what should be included in the assessment.

The final document will be available to the community. We hope that it will be a data resource to help meet grant deliverables and apply for new funding opportunities. We also hope this tool can increase collaboration to improve health outcomes in the Lubbock area. With that in mind, please complete the following survey to help ensure that the assessment is of maximum benefit to our partners and community.
